As part of Medicine on Blackwood’s philosophy of serving as your ‘Medical Home’, we take our responsibility for your ongoing, comprehensive, whole-person medical care very seriously.

Our patients and their families enjoy a continuing relationship with a particular GP and their colleagues with support from a practice team and other clinical services in the ‘medical neighbourhood’.

The medical home coordinates the care delivered by all members of your care team and, when required, this may include hospital inpatient care. At Medicine on Blackwood our focus as your medical home is to ensure you experience fully integrated health care.
